State run mass vaccination sites are now welcoming walk-ins in addition to pre-booked appointments. All remaining vaccination locations are by appointment only.

Appointment information for non-state run providers can be found below. Residents who need assistance or lack online access can call the toll-free IDPH hotline at 833-621-1284 to book an appointment.

Please bring documentation to your appointment to show that you are eligible for vaccine as part of a priority group. Providers may NOT ask individuals to provide documentation regarding their immigration status.
